on my behalf for being a Judge in Research Colloquium which being
conducted last June 20, 2021 Via Zoom organized by College of Arts and Sciences.
Due to unforeseen circumstances, I am unable to personally attend the event and
receive the honorarium.

paperwork or documentation required for this purpose.
